Highlights • Reserve cells nested in the endocervical epithelium are alternative HPV targets.• Two distinct host cells of the cervix can be infected and transformed by HPV.• Most clinical observations are better explained by two target cells for HPV.• The alternative disease progression model is linked to the reserve cell pathway.
