Abstract

standardized protocols: (1) a High-Risk Medication Protocol using weight-based standardized dosing tools, organ function-guided dose adjustments, and standardized reversal pathways integrated with sedation/analgesia scoring scales; (2) a Dose Verification Protocol to address dose calculation and administration errors through structured double-check processes which are adaptable to electronic or manual systems; and (3) a Medication Reconciliation Protocol targeting omission, duplication, and transition errors through standardized review tools at admission, care transitions, and discharge. Each protocol includes comprehensive implementation guidance for adapting to local workflows, personnel roles, and documentation systems. Conclusion: Our three-priority framework, using standardized protocols and adaptable tools, can address most ICU medication errors identified in national pharmacovigilance data. By focusing on high-impact preventable errors, this approach supports practical improvements in medication safety across diverse ICU settings and is ready for local implementation and future multicenter testing.
