Abstract

Background: Interstitial lung disease (ILD) is a heterogeneous group of diffuse parenchymal lung disorders with significant morbidity. HRCT-WARRICK scoring and the GAP prognostic index are established tools for assessing disease severity and predicting outcomes in ILD. Objectives: To correlate the HRCT-WARRICK score with pulmonary function parameters (FVC, FEV₁, FEV₁/FVC, DLCO, 6MWT) and the GAP prognostic score in ILD patients. Methodology: A hospital-based cross-sectional study was conducted at CHRI, Kelambakkam, from March to April 2025, enrolling 50 adult ILD patients. HRCT-WARRICK scoring, spirometry, DLCO, 6MWT, and GAP scoring were performed for all participants. Results: The study included 31 males (62%) and 19 females (38%), with a mean age of 58.36 ± 9.65 years. The mean WARRICK score was 13.7 ± 5.81. Significant negative correlations were found with FEV₁ (r = −0.376), FEV₁/FVC (r = −0.336), DLCO (r = −0.547), and 6MWT (r = −0.445). A strong positive correlation was observed with GAP score (r = 0.739) and GAP stage (r = 0.778), all statistically significant (p < 0.05). Conclusion: The HRCT-WARRICK score effectively reflects functional impairment and prognostic severity in ILD, supporting its integration with PFT and GAP scoring in routine clinical evaluation.
