Le cinque raccomandazioni Green Choosing Wisely del Gruppo di Studio Evidence-Based Laboratory Medicine della Società Italiana di Patologia Clinica e Medicina di Laboratorio

The Choosing Wisely (CW) initiative, through its lists of five recommendations, addresses both physicians and patients with the aim of promoting a partnership focused on the appropriateness of care, including the recently introduced Green Choosing Wisely perspective, dedicated to sustainability issues. This framework inspired the Evidence-Based Laboratory Medicine Study Group (GdS EBLM) in developing recommendations on sustainability in Laboratory Medicine on behalf of the Italian Society of Clinical Pathology and Laboratory Medicine (SIPMeL). These recommendations address, on the one hand, key components of the Total Testing Process (TTP) and, on the other, issues related to the organization, management, and mission of Laboratory Medicine. The objective is to support Laboratory Medicine in contributing, through concrete and measurable actions, to the development of a sustainable healthcare system, promoting the appropriate and efficient use of resources from ecological, social, and economic perspectives, while maintaining high standards of quality in services provided to patients and clinicians.• Do not perform routine laboratory tests or screenings without a clear clinical question and within an inappropriate retesting interval;• do not acquire reagents, instrumentation, and/or laboratory supplies without considering their environmental impact, while maintaining diagnostic efficiency;• do not use the paper report when a dematerialized report or electronic health record is available;• do not engage in automatic or unconscious behaviors that lead to the waste of reagents, the use of nonessential single-use plastics, improper waste management, or energy waste, in compliance with the laboratory’s Quality System;• do not overlook advocacy for “Green Laboratory” initiatives, a key element in raising awareness among institutions, companies, citizens, and patients toward a sustainable healthcare system. The laboratory must not limit itself to communicating internally its choices, awareness initiatives, results, and sustainability goals, but must also address patients, institutions, suppliers, and the broader community.
