Emergent laparoscopic pancreatectomy with splenectomy for a primary pancreatic lymphoma with spontaneous splenic rupture: a case report

Primary pancreatic lymphoma (PPL), which is defined as lymphoma originating from pancreatic lymphoid tissue or infiltrating lymphocytes, is a rare clinical manifestation with non - specific symptoms. Surgical resection is not regarded as the first - line treatment for PPL; however, it may be indicated for the management of complications or the clarification of diagnosis in challenging cases. Emergent laparocospic distal pancreatectomy with splenectomy (E-LDPS) has not been reported to treat emergent conditions of PPL. Herein, a 63 - year - old male who underwent emergency laparotomy due to acute intra - abdominal hemorrhage is presented. Intraoperative findings led to a radical LDPS due to suspected splenic rupture. Subsequently, histopathological examination confirmed primary diffuse large B - cell lymphoma of the pancreas. The patient completed a full - course chemotherapy regimen and achieved complete remission, with no recurrence observed during the follow - up period. This case highlights the potential significance of early minimally invasive surgical intervention in the management of PPL when emergent complications occur in high volume center.
