Abstract

OBJECTIVE To describe perioperative nursing management strategies and clinical outcomes in patients undergoing hybrid repair for complex Stanford type B aortic dissection.

METHODS This single-center retrospective descriptive observational study included 98 patients with acute Stanford type B aortic dissection who underwent hybrid supra-aortic branch bypass combined with thoracic endovascular aortic repair between January and December 2024. Standardized perioperative nursing protocols were implemented, focusing on hemodynamic stabilization, spinal cord ischemia prevention, contrast-induced acute kidney injury prevention, and postoperative monitoring. Clinical outcomes and perioperative complications were analyzed descriptively.

RESULTS Hybrid procedures were technically successful in all patients. The mean hospital stay was 19.58 ± 6.29 days. Ninety-six patients (98.0%) were discharged in stable condition. Spinal cord ischemia occurred in one patient (1.0%) and resolved after cerebrospinal fluid drainage and medical management. Contrast-induced acute kidney injury occurred in two patients (2.0%) and resolved with hydration therapy. No patients experienced stroke, permanent neurological deficit, or graft-related complications. Wound complications were minimal and successfully managed.

CONCLUSION Hybrid repair for complex Stanford type B aortic dissection is a safe and effective treatment strategy when supported by structured perioperative nursing management. Standardized nursing protocols play a critical role in maintaining hemodynamic stability, preventing major complications, and promoting postoperative recovery.
