Abstract
ABSTRACT Introduction Bacterial pneumonia remains a leading cause of morbidity and mortality. β-lactam antibiotics have represented the backbone of treatment for community-acquired and hospital-acquired pneumonia. However, the emergence and global spread of antimicrobial resistance have compromised the efficacy of many established β-lactams, creating major therapeutic challenges, particularly in severe and nosocomial infections. Areas covered This narrative review summarizes the evolving role of β-lactam antibiotics in the treatment of bacterial pneumonia, with particular attention to resistance mechanisms in key respiratory pathogens. The manuscript examines trends in β-lactam resistance among Gram-positive and Gram-negative bacteria and discusses the clinical relevance of newly developed β-lactams and β-lactam/β-lactamase inhibitor combinations. Recently introduced agents, including novel cephalosporins, monobactam-based combinations, and carbapenems, are reviewed with respect to their microbiological activity, clinical evidence, and potential role in community-acquired, hospital-acquired, and ventilator-associated pneumonia. Expert opinion β-lactams continue to play a central role in pneumonia management, but their effectiveness increasingly depends on appropriate patient selection, optimized dosing, and antimicrobial stewardship. While new agents have expanded therapeutic options against multidrug-resistant pathogens, none provide universal coverage. Future progress will rely on improved diagnostics, a deeper understanding of resistance evolution, and the integration of innovative therapeutic strategies to preserve the clinical utility of β-lactams.
