Abstract

The objective of this study is to analyze the evolution, structure, and effectiveness of the Operational and Administrative Service (ORS) of the Moscow City Health Department (DZM), using the liquidation of the consequences of the Moscow metro accident on July 15, 2014, as a case study. Materials and methods. The work is based on a retrospective analysis of operational documents, reporting materials, and regulations of the ORS DZM and its predecessor, the Control and Administrative Service. Research results and analysis. A three-level operational model of the ORS (daily operations, heightened readiness, and emergency mode) is described. Using the specific incident as an example, a detailed action algorithm is presented: from signal reception to coordination of medical evacuation and the creation of a “hotline.” The analysis confirmed that the ORS functions as a central hub for information exchange in two key areas: ensuring intradepartmental communication (DZM management — medical organizations — mobile teams of the Center for Emergency Medical Care and interdepartmental interaction (with the Ministry of Emergency Situations, the operational service of the Moscow Mayor’s Office, and other city services). Conclusion. The experience of the ORS DZM during a large-scale man-made emergency proved its effectiveness as a sustainable management system capable of ensuring the continuity of medical care without staff expansion. The proven ORS model is of practical interest and can be adapted to the healthcare systems of other metropolitan cities.
