Mature Benign Cystic Teratoma of Retrouterine Pouch of Douglas

Mature benign cystic teratoma is a commonly encountered ovarian tumor, whereas the occurrence in the pouch of Douglas is relatively uncommon and the specific etiology remains unknown. The case report brings to fore a woman in her early twenties with lower abdominal pain suggestive of complex echogenic mass in pouch of Douglas on ultrasonography. The patient underwent laparoscopic excision of the mature cystic teratoma of the pouch of Douglas. Macroscopically, a 5.9 cm × 4.3 cm cystic tumor that emerged from the pouch of Douglas revealed that the mass was visibly detached from both the ovaries. From a microscopic perspective, the cyst was a fully developed cystic teratoma that did not originate in the ovaries. Postsurgery, 3-month follow-up showed full recovery of the patient with significant clinical improvement and USG pelvis revealed no recurrence at the primary site.
