Abstract

Цель исследования — выявить наиболее безопасный препарат для седации при эндопротезировании коленного сустава, с минимальным влиянием на когнитивные функции и гемодинамику у пациентов старческого возраста. В проспективное контролируемое рандомизированное исследование вошли 78 пациентов старческого возраста (75–90 лет) с диагнозом первичного и вторичного гонартроза III–IV стадии. Операция — эндопротезирование коленного сустава, анестезия во время операции — комбинированная спинально-эпидуральная. Рандомизация была произведена на две группы: в интраоперационный период в качестве седации в одной группе применяли пропофол, в другой — дексмедетомидин. Степень глубины седации оценивали при помощи шкалы седации Ramsey, для мониторинга глубины медикаментозного сна применяли аппарат биспектрального индекса «BISмониторинг». Когнитивный статус оценивали при помощи шкалы депрессии Гамильтона при госпитализации пациента и на 2-е сутки после оперативного вмешательства. Интраоперационную гипотензию выявляли чаще в группе пациентов с седацией пропофолом, а интраоперационную брадикардию чаще регистрировали в группе пациентов с седацией дексмедетомидином. Контролируемая BIS-мониторированием седация как пропофолом, так и дексмедетомидином с показателями 82–88 % обеспечивала уровень в 3–4 балла шкалы седации Ramsey и позволяла уменьшить развитие когнитивных нарушений согласно полученным результатам шкалы депрессии Гамильтона у пациентов при эндопротезировании коленного сустава. The aim of the study was to identify the safest drug for sedation during knee arthroplasty, with minimal impact on cognitive functions and hemodynamics in elderly patients. The prospective, controlled, randomized study included 78 patients diagnosed with primary and secondary gonarthrosis stage III–IV, elderly patients (75–90 years). Surgery — knee arthroplasty, anesthesia during surgery — combined spinal-epidural anesthesia. Randomization was performed into 2 groups — propofol was used as a sedative during the intraoperative period and dexmedetomidine was used as a sedative during the intraoperative period. The degree of sedation depth was assessed using the Ramsey sedation scale, to monitor the depth of drug-induced sleep, we used a bispectral index device — BIS monitoring. Cognitive status was assessed using the Hamilton Depression Scale at admission and on day 2 after surgery. Intraoperative hypotension was more common in the group of patients with propofol sedation, and intraoperative bradycardia was more common in the group of patients with dexmedetomidine sedation.
