Tourniqet Technique in Caesarean Myomectomy: A Novel Approach to Prevent Massive Haemorrhage in Multiple Uterine Fibroids: A Case Report

Simultaneous caesarean section and myomectomy in patients with multiple uterine fibroids poses a high risk of bleeding. The tourniquet technique offers a practical solution to minimise blood loss, making surgery safer and easier. Here, we report successful caesarean myomectomy using the tourniquet technique in a patient with multiple uterine fibroids. The patient was a 32-year-old primigravida with a 9-year infertility history who developed pregnancy after uterine rotation caused by a large fundal fibroid (12×12 cm). She underwent a caesarean section with myomectomy using the tourniquet technique. Twelve intramural fibroids were successfully removed with minimal blood loss (300 ml) in 90 minutes. No massive hemorrhage occurred, and the patient recovered without complications. Postoperative imaging confirmed complete fibroid removal with preserved uterine morphology. The tourniquet technique is a safe, effective, and accessible method for haemorrhage control during caesarean myomectomy, particularly in resource-limited settings.Case Presentation: We present a unique case of a 32-year-old primigravida with a 9-year infertility history who developed pregnancy after uterine rotation caused by a large fundal fibroid (12×12 cm) relieved pressure on the uterine cavity. She successfully underwent a caesarean section with myomectomy using the tourniquet technique, removing 12 intramural fibroids with minimal blood loss (300 ml).Conclusion: The procedure was completed successfully in 90 minutes with excellent haemostasis. No massive haemorrhage occurred, and the patient recovered without complications. Postoperative imaging confirmed complete fibroid removal with normal uterine morphology. The tourniquet technique represents an effective, safe, and accessible method for haemorrhage control during caesarean myomectomy, which is particularly beneficial in resource-limited settings.
