Abstract
Objective To investigate perinatal outcomes of twin pregnancy.Methods Perinatal outcomes of 658 pregnancies with twins hospitalized and born at Beijing Obstetrics and Gynecology Hospital during 2002 to 2005 were retrospective analyzed to compare the influence of type of twin,delivery mode, gestational week,birth time interval between twins,as well as specific complications of twin pregnancies on their outcomes.Results Pefinatal fatality of monzygous twins was 53.69 per thousand(16/298),higher than that of dizygous twins(27.11 per thousand,9/332),with U=2.35,P0.05.Occurrence of all complications was lower in twins born by Caesarean section than that by vaginal birth,regardless of their fetal presentation.Rate of admission to neonate intensive care unit and occurrence of neonatal asphyxia were lower in twins born at an interval equal to or more than 15 minutes(50.0 percent,34/68 and 10.3 percent, 7/68,respectively)than those born at an interval more than 15 minutes(64.3 percent,18/28 and 21.4 percent,6/28,respectively),P0.05.Perinatal fatality was higher in twins with different development (birth weight).Perinatal fatality in dizygous twins(12.5 percent,2/16)was lower than that in monozygous ones(14.3 percent,6/42),and that in twins with different development(birth weight)(0.6 percent, 2/316)was lower than that with same development(1.2 percent,3/256),with χ~2=16.944,P0.01. Occurrence of fetal death and neonatal asphyxia in twins with twin-twin transfusion syndrome(both with 21.4 percent,3/14)were higher than those in twins without complications(1.7 percent, 11/644 and 3.0 percent,19/644,respectively),with an exact probability of less 0.002 and 0.009,respectively. Conclusions Type of twin pregnancies should be confirmed as early as possible and more attention should be paid to antenatal care for monozygous twins to detect and treat for those with twin-twin transfusion syndrome for birth weight discordant twins in time.
