The CT and MRI imaging review analysis of unexpected gallbladder cancer

Objective To improve the accuracy rate of gallbladder carcinoma preoperative image diagnosis,and reduce the incidence of unexpected gallbladder cancer( UGC). Methods Retrospective analysis of 19 gallbladder carcinoma patients who performed with laparoscopic cholecystectomy( LC) in our hospital from 2000 to 2012,the CT and MRI iconography data and postoperative pathological diagnosis data were compared to analyze the reason of the occurrence of UGC.Results In 19 cases of UGC,12 out of 19 were missed diagnosis. Among them,7 cases were missed diagnosis by CT,7cases were missed diagnosis by MRI and 2 cases were missed diagnosis both by CT and MRI. For the iconography type,12 cases were infiltration cancer,of these 8 cases showed the cystic wall localized thickening,2 cases showed diffuse thickening and 2 cases could not be displayed. 4 cases were nodules cancer,among them 3 cases were inhomogeneous enhancement and fuzzy contour,adjacent to the gallbladder wall thickening. 3 cases were mass cancer,all were MR plain scanning with uniform signal and slightly longer T1 / T2 signal,and all were accompanied by multiple stones. Conclusion Standard imaging techniques and improved understanding on gallbladder cancer signs may helpful to reduce the incidence of UGC.
