Effect of dexmedetomidine for prevention of uterotonic adverse reaction in cesarean section

Abstract

Objective To explore the prevention action of dexmedetomidine for uterotonic adverse effects in cesarean section. Methods Seventy cases puerpera with lower uterine segment cesarean section maternal to full term,were randomly divided into control group and dexmedetomidine group,with 35 cases in each group. After delivery of the fetus,all the patients received intravenous infusion of 20 u oxytocin while carboprost 250 μg was injected into the myometrium simultaneously. In dexmedetomidine group,after a loading dose of dexmedetomidine 0. 1 μg·kg- 1,dexmedetomidine was infused at a speed of0. 4 μg·kg- 1·h- 1starting from one minute prior to carboprost injection until the end of surgery,while the equal volume of normal saline was given in control group. Intraoperative situation,adverse effects rate,observer's assessment of alertness / sedation( OAA / S) score of the end of the surgery and two hours after operation,initial time of lactation of two groups were recorded. Results There was no significant difference in the operation time,amount of bleeding,infusion quantity and urine volume during operation,OAA / S score of the end of the surgery and two hours after operation,initial time of lactation between the two groups( P 0. 05). But compared with control group,the incidence of nausea,vomiting,facial flushing,dyspnea,hypertension and tachycardia was significantly lower than that in dexmedetomidine group( P 0. 01). Conclusion Dexmedetomidine can prevent the uterotonic adverse effects after fetal delivery,and has less effect on OAA / S score and initial time of lactation. 。
